A Patient-Centered Medical Home (PCMH) is a trusting partnership between a doctor-led health care team and an informed patient. In a medical home, the practice is organized around the patient – communication is based on trust, respect and shared decision-making.
As a PCMH, Thunder Bay Community Health Service will work with you to help identify ways to improve your health such as creating a care management plan, identifying and helping you achieve healthy goals, and coordinating your care with other health care organizations such as specialists or hospitals.
Thunder Bay Community Health Service Host Open House at the Atlanta Clinic Site
On August 29, 2019, Thunder Bay Community Health Service, Inc. held an open house at their Atlanta Health Center to showcase our 3,400 sq. ft. dental expansion and renovation of our pharmacy and medical space. In 2016, TBCHS was the proud recipient of a $1,000,000 Health Infrastructure Investment Program federal grant from the Health Resource Services Administration to make facility enhancements to increase patient capacity.More News Stories
