We are providing this notice to inform the community that we have experienced a breach of several patients’ health information. This breach involved an employee at Thunder Bay Community Health Service Inc. who inappropriately accessed personal health information. Due to the historic nature of the records that were accessed, we are posting this publicly in case the addresses we have on file are no longer the addresses of those who were affected by this breach.
On 11/19/2024 a verbal report informed us that an individual’s personal health information may have been accessed by the TBCHS employee, this prompted us to investigate the issue and out of caution we determined to extend our investigation into other patient records that may have been reviewed unnecessarily. We have determined the employee had used their medical record credentials to inappropriately access health information out of curiosity. We investigated this situation between 11/19/2024 and concluded on 12/05/2024. According to our investigation the Health Center employee accessed records on several dates beginning 11/3/2023 and ending on 4/23/2024 The records contained:
- Clinical information, including diagnosis/conditions, lab results, medications, and other treatment information
- Demographic information, including name and date of birth
